By completing this application, you may be considered for the 2017 ITF Young Seniors cup team selection, provided you have otherwise met the team selection requirements. The team selection will be held November 6, 2016.
This application is due by October 7th, 2016. The link to this survey will no longer be available at 11:59 pm pacific time.  
**If you have not played on a team in the past two years, in addition to completing this application you will need to email me a scanned copy of your US passport to mr@usta.com.
